Community Health Workers are in the frontlines of our communities, helping individuals, families, and groups to navigate complexity, overcome barriers and connect to necessary health and social service resources. Our Mission is to support Community Health Workers (CHWs) in their ever-changing communities and to raise awareness of the CHWs valuable contributions. Our strategy for pursuing this mission is: Awareness The “I am a CHW” Campaign and Partnerships, which give CHWs a platform to share their stories and raise awareness about the value they add in their communities. Network To create a broader network of CHWs that provides opportunities for strong peer-to-peer connections, advocacy, and continued education. Outreach To promote the C.O.R.E. Project that helps community members connect to necessary resources and overcome barriers. Our Vision is to grow a National Community that celebrates CHWs and creates visibility for their contributions.
